LABORATORY/PHYSICAL EXAM REQUIREMENTS
(Incoming BSN/Midwifery 1st Year Including Transferees)
1. LATEST LABORATORY AND X-RAY;
- CBC
- Urinalysis and Fecalysis
- HEPATITIS B SCREENING or HBsAG
- CX-RAY-PA View

2. PHYSICAL EXAMINATION
Instructions:
- Undergo the laboratory and X-ray procedure at any hospital/laboratory of choice.
- Get the results and proceed to the Doctor's clinic of choice for the physical exam.
- Secure the Doctor's recommendation (Medical Certification) together with the laboratory and x-ray results and submit them to the school clinic for the next step.
